If you encounter a problem with submitting the form, please send your application without attachments and send photos to: reklamacje@dasag.eu.

Lack of sufficient data will result in the rejection of the application.

The form should be accompanied by:
- a photo of the product damage / defects,
- a copy of the sales document / delivery note,
- confirmation of payment for the complained goods,
- photos of the product label, quality control label (if possible).

I consent to the processing by PROBET-DASAG Spółka z o.o. with its registered office at: 68-100 Żagań, 4-6 Fabryczna Street, entered into the Register of Entrepreneurs of the National Court Register under KRS number 0000136496, my personal data provided in this complaint form, in order to send a complaint application. At the same time, I agree that Probet Dasag Sp. z o. o. will send electronic correspondence to the above e-mail address. In the event of technical or formal obstacles preventing the sending of correspondence by electronic means, I undertake to accept correspondence in paper form. At the same time, in the event of a change of e-mail address, I undertake to notify Probet-Dasag Sp. z o. o. of the new address in writing.
To send a report, consent is required. Before giving consents, please read the information clause – Article 13 of the GDPR .
